Family Maintenance And Spine Care Plus Programs The purpose of each of these programs is to provide the very finest Chiropractic care possible and to make this care affordable to everybody, regardless of their financial abilities. Dr. J. L. Hoover (founder) developed the Family Maintenance Program years ago so parents and their children could have the benefit of regular chiropractic care. It has been his "pet program" over the years. He started this program because he had a large family and knew how important it was for his children to grow up with Chiropractic care. He also realized that he could not afford to have his own children checked as often as he felt it necessary if it were not for such a program or if he had not been a Chiropractor. This program makes it possible for children to have care on a regular basis, and often for less than the cost of having their haircut or a trip to McDonalds. "Spine Care Plus" is an extension of our maintenance program. It is our answer to the HMO/PPO dilemma. This special program allows new patients to enroll in our maintenance care plan directly. It is for patients needing "intensive care" or "on going care" in which our regular primary fees might cause a hardship. "Spine Care Plus" bridges the gap between primary and maintenance care. It is a pay as you go plan designed for patients who either do not have insurance coverage for Chiropractic care or do not wish to use their insurance. Many companies have changed to HMO's/PPO's etc. which greatly limits patients from choosing their own doctor or health care method. These so-called "managed care" programs limit the amount of care you can receive even when they approve a doctor or service. "Spine Care Plus" offers patients unlimited care at a greatly reduced fee. It allows you to manage your own care! When you consider the long years of schooling required to be a "Doctor of Chiropractic", the expensive office equipment necessary, the rather large office facilities to maintain, office staff, liabilities, etc. you begin to realize the tremendous bargain these programs are in today's world. It may also make you wonder "why" and "how" we do it. The "why" part is easy for us to answer. We simply feel that every person on earth needs their spine checked on a regular basis from birth throughout their entire life. The "how" part is difficult to accomplish in these inflated times. It took much planning. It is based on running a very efficient office and on seeing large numbers of patients. We simply must be willing to work more hours and see more patients than an average office with higher fees. This is our responsibility in bringing these programs to you. In addition to our responsibility it is necessary to ask you to meet the following very simple, but important requirements. 1. You Must Read And Sign A "Request For Maintenance Care Form." Patients are automatically enrolled in our "Regular or Primary Care Program" They may choose to change to either Maintenance or Spine Care Plus by reading and signing a special request form that fully explains our Terms Of Acceptance. 2. You Must Be Actively Under Care. This means you must see us one visit per month or more when enrolled in the "Family Maintenance" program. You must see us at least one visit within a three-month period to remain active in the "Spine Care Plus" program. If you see us less than this, your care is not considered maintenance and you are automatically transferred back to our regular or "Primary Care" program.
You may renew your maintenance status on
your first visit back to us by signing a "Maintenance Renewal Request Form."
The reason for this particular requirement is that we have found that patients who take care of their health by seeing us on a regular basis stay healthy and are much easier for us to see. In some cases it takes us longer to put a patient, who has neglected his health, on an adjusting table than it takes us to see an entire family who takes care of their health. We are able to pass these savings to you. 3. You Must Pay For Each Visit At The Time It Is Made. This saves us the cost of expensive bookkeeping and billing. It is a very important factor in making these programs work. We, at best, barely break even on these programs. This requirement must be met! For your convenience we accept both Master Card and VISA. 4. No Forms Of Any Kind Are Filled Out By "Spine Care" For These Visits. You are informed at the time you request this care that most insurance companies do not pay for maintenance care. A very large part of our office expense is in filling out forms, answering special request, billing, waiting on payment and in some cases never receiving payment. This requirement of our maintenance program saves us much personal time and expense. You will be given an itemized receipt upon request at the time payment is made. If we continue to work our long hours and each of you are willing to abide by these few simple, but important rules, we will be able to continue these wonderful programs. Keep in mind that it has always been our policy to see all patients regardless of their financial ability to pay. If our normal procedures cause a hardship on any patient, he is invited to discuss it with our business office and work out other arrangements. In summary, the" Family Maintenance" and "Spine Care Plus" programs are our favorite way to practice. It brings you the very best of Chiropractic care for often less than half the cost of our very modest regular or "Primary Care" program. |
